How do I surrender my Universal Life Insurance policy?

We're sorry to lose you as a customer but understand the need to surrender. Please fill out the "Request For Disbursement _ Life" (ADMIN 5586) form and mail or fax it to our service center. NY residents please fill out please fill out  "Request For Disbursement" (NYAD3047).
